Your Initial Appointment
At Crestview Endodontics, your first visit is an opportunity for us to get to know you and understand your dental needs and concerns. From the moment you walk through our doors, our friendly staff will greet you with warmth and professionalism, ensuring you feel comfortable and at ease. During your initial consultation, we’ll conduct a thorough examination, discuss any issues or goals you have, and create a personalized treatment plan tailored to achieve your optimal dental health and enhance your smile.
Please bring the following to your consultation:
- Your referral slip and any relevant X-rays
- A current list of medications you are taking
- Completed medical or dental insurance forms, if applicable, to help us process claims efficiently

Patients under 18 must be accompanied by a parent or guardian.
Medical Considerations
Please inform our office if you have any medical conditions that could affect treatment (such as diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.), or if you are taking medications like heart medications, aspirin, anticoagulants, or require pre-medication before dental procedures. This information helps us ensure your safety and provide the best care possible
Financing & Insurance
Since most patients are seen for a single visit, payment is collected at the time of your appointment. We accept cash, checks, and major credit cards. CareCredit financing is also available to help make treatment more manageable.
