Endodontic Retreatment
Saving Teeth When Previous Treatment Needs a Second Look
Sometimes, a tooth that has undergone root canal therapy may not heal as expected or may develop new problems over time. Endodontic retreatment is a specialized procedure designed to address these issues and preserve your natural tooth.
The failure of an initial root canal can happen due to new problems or issues that could not be fully addressed during the first procedure.
Why the Initial Treatment May Fail:
- Incomplete Cleaning: The most common cause is that the initial procedure failed to fully locate and disinfect all the microscopic canals.
- Access Issues: The initial treatment was blocked by complicated or narrow canal anatomy that was not fully visible without the advanced techniques (like microscopic endodontics) used in retreatment.
- Delayed Sealing: The placement of the permanent crown or filling was delayed after the root canal, allowing the interior of the tooth to become re-contaminated by saliva and bacteria.
- Inadequate Restoration: The permanent crown or filling placed on the tooth after the root canal fractured, cracked, or loosened. This allows bacteria to leak into the sealed canal system.
- New Decay: New decay develops beneath the crown or filling, eventually reaching the root canal filling material and leading to renewed infection.
Why a Retreatment is Needed
Retreatment is necessary to eliminate infection and pain and prevent the need for extracting the tooth.
- Eliminate Persistent Infection: The primary goal is to thoroughly clean the complex, microscopic anatomy that was missed or re-infected after the first procedure. This eliminates the source of pain and swelling.
- Save the Natural Tooth: Endodontists are specialists in preservation. Retreatment offers the best way to save the natural tooth structure, which is always preferable to extraction. Saving the tooth prevents shifting, preserves jawbone density, and avoids the need for expensive bridges or implants.
- Correct Structural Defects: During retreatment, the specialist can remove old filling materials, place new seals, and repair internal damage (like a fractured root) or previous errors that may have compromised the long-term prognosis.
- Long-Term Function: Successfully retreated teeth can often last a lifetime, restoring the tooth to full function for biting and chewing.
Retreatment is essentially giving your tooth a second chance at a permanent, healthy solution.
